The filter definition uses an older syntax in which filters could be linked to multiple forms. The new definition allows only a single form link. The filter is loaded, but only with access to the first form in the list of forms. Ask the administrator to restructure the filter definition to see a single form. If you need filters with the same actions, create multiple filters.
